Job overview

To support the effective delivery of patient care in GP practices within a defined Primary Care Network (PCN) by utilising pharmaceutical skills to embed the principles of medicines optimisation within the wider practice teams, ensure safe patient care through regular review and monitoring of patients based on clinical need and support demand management through consultation with patients as appropriate.

Qualifications

You will be a qualified Pharmacist with experience within Primary Care and have the following skills / qualifications:

Hold or working towards Masters Degree in Pharmacy or equivalent

Registered Pharmacist with GPhC

Independent prescriber

Member of the Royal Pharmaceutical Society (RPS)

Job responsibilities

  • To work autonomously as an independent prescriber using extensive experience and in depth knowledge to be directly responsible for care and prescribing of defined cohort(s) of patients
  • To deliver and develop pharmacy led medicines optimisation working with general practice and the wider multidisciplinary team, ensuring practice remains up to date

 

 

  • To lead and promote, support and facilitate medicines optimisation for specialist area in the wider medicines agenda ensuring best value, reduced harm and improved outcomes.
  • To lead and deliver evaluation and audit of service delivery for specialist area in order to ensure a high quality service and to further develop provision.
  • To work to the principles of Medicines Optimisation and relevant up to date national guidelines and evidence base. To support the Prescribing Lead with the medicines agenda within the practice
  • Lead on the medicines management of a cohort of patients (e.g. cardiovascular, frailty, pain, mental health). Has up to date knowledge of complex therapeutic and drug related issues within specialist area
  • Advise on medicines and polypharmacy, suggesting alternatives and helping to reduce wastage within the practice
  • Manage the transfer of care from different care settings (hospital/care homes) including medicines reconciliation, obtaining specialist supplies and liaison with local pharmacies.
  • Support a program of medicines reviews within specialist interest area and generalist reviews for complex patients
  • Help develop bespoke integrated pharmaceutical care plans with individual patients
  • Provide education and training for clinicians and the prescription team on complex prescribing problems Rationalise repeat prescription lists to avoid wastage and duplication and promote implementation of the repeat dispensing service Promote self-care, support health promotion and wellbeing advice where appropriate
  • Lead on analysis and interpretation of complex health and drug related information and critical appraisal of published literature
  • Interpret and implement national and local medicines optimisation programmes as directed
  • Identify patients’ safety through the use of medicines including MHRA alerts, product withdrawals and emerging evidence from clinical trials.
  • Run clinics where medicines are the main interventions e.g. anticoagulant monitoring clinics.
  • Conduct care planning including mental health, admission avoidance schemes and End of Life patients.
  • Undertake minor ailments triage and direct patients based on clinical need to capture relevant patient health information at all consultations including contributing to the quality outcomes framework.
  • Monitoring patients with complex long term conditions such as hypertension, diabetes and COPD, ensuring that medication is optimised, titrated, monitored and complied with.
  • Providing clinical advice and expertise on medication.
  • Establishing ongoing professional relationships with individual patients.
  • Act across the interface liaising with other providers i.e. secondary care, community Pharmacy and care/nursing homes as necessary to ensure safe transfers of care.
  • Deliver on and/or oversee relevant aspects of the PCN Directed Enhanced Service (DES).
  • Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists
  • Be part of a professional clinical network and have access to appropriate clinical supervision.
  • Support the PCN Clinical Director as necessary
  • Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates.
  • Review prescribing against local and national targets and engage with the wider prescribing team to rectify.
  • Carry out structured medicines optimisation reviews in care homes
  • To contribute to the reduction of medicine wastage within practices across the PCN
  • Patient facing care home medication reviews

 

 

  • Undertake clinical medication reviews with patients and produce recommendations for the senior clinical pharmacist, nurses or GPs on prescribing and monitoring.
  • Work with care home staff to improve safety of medicines ordering and administration.
  • Patient facing domiciliary clinical medication review
  • Undertake clinical medication reviews with patients and produce recommendations for the senior clinical pharmacists, nurses and GPs on prescribing and monitoring.
  • Attend and refer patients to multidisciplinary case conferences.
  • Management of common/minor/self-limiting ailments
  • Managing caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence.
  • Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate.
  • Patient facing medicines support
  • Provide patient facing clinics for those with questions, queries and concerns about their medicines in the practice.
  • Telephone medicines support
  • Answers relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines. Suggesting and recommending solutions.
  • Providing follow up for patients to monitor the effect of any changes.
  • Unplanned hospital admissions
  • Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
  • Put in place changes to reduce the prescribing of these medicines to highrisk patient groups.
  • Management of medicines at discharge from hospital
  • To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.

 

 

  • Set up and manage systems to ensure continuity of medicines supply to highrisk groups of patients (e.g. those with medicine compliance aids or those in care homes).

Desirable

  • Peer credentialed by the RPS Faculty (at expert
  • level 2 or greater)
  • Diploma or Certificate or MSc in Clinical
  • Pharmacy

Experience

Essential

  • Prior experience in Primary Care
  • Driving License
