We are looking for pharmacists to join our NEW Norfolk & Waveney STAR programme. You will work in a portfolio role across two pharmacy sectors with the unique opportunity to work as a newly qualified pharmacist in GP. STAR pharmacists will be enrolled on the Primary Care Pharmacy Education Pathway (PCPEP) learning pathway, Independent Prescribing (IP) and can access optional education modules to support a personal development plan.

The STAR programme supports you to work in multiple sectors of pharmacy, access key training programmes to develop in your career, create your e-portfolio and access supervision. In phase 1 of the programme in 2023 you will work in general practice and community pharmacy. In phase 2, we aim to offer portfolio roles in other sectors such as: hospital practice, health and justice, and mental health.

STAR is open to pharmacists who have registered with the GPhC in the last three years, or who are currently completing their Foundation year. Phase 1 employment will start in Summer 2023.

The STAR programme will kick start your career with multi-sector experience, education to start your prescribing and advanced or enhanced practice journey. You will contribute to integration across primary care and deliver improved patient outcomes.

Detailed job description and main responsibilities

The precise role of each clinical pharmacist will vary slightly depending upon the needs and expectations of the PCN. Roles and responsibilities for the clinical pharmacist will develop and grow as they complete their primary care training, and it is expected that pharmacists will deliver all the elements of the Direct Enhanced Service (DES) as they progress through the Primary Care Pharmacy Education Programme (PCPEP) course. 

The role responsibilities will include:

  • Clinically assess and treat patients using expert knowledge of medicines for specific disease areas
  • Contribute to accurate medicines reconciliation upon transfer of care
  • Support colleagues and patients with medication queries and ensuring medications are monitored in line with national guidance e.g., NHS Specialist Pharmacy Service (SPS)
  • Take responsibility for the care and management of patients with chronic diseases (within your areas of expertise and/or interest)
  • Undertake clinical medication reviews to proactively manage people with complex polypharmacy. Focusing especially on the elderly, people in care homes, those with multiple co-morbidities (frailty, COPD, and asthma) and people with learning disabilities or autism
  • Provide specialist expertise in the use of medicines, while helping to address both the public health and social care needs of patients
  • Undertake clinic audits to improve patient care and identify practice improvement points
  • Provide leadership on person-centred medicines optimisation (including ensuring prescribers in the practices conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement

Hybrid Working

The ICB operates a flexible approach to working. It is expected the majority of our staff will work from a combination of home and office (where possible). For staff designated in a hybrid role they should have a safe and suitable place to work with Broadband access and space for a desk, chair and monitors. At interview a discussion will take place between the recruiting manager and the candidate to ensure they are clear on what is expected.
